Greater Cincinnati Cover the Uninsured Month 2010 Events

Enrollment Blitz

Volunteers will be stationed at the Kroger stores below to help you qualify for medical coverage.  Come on Saturday April 10 or April 17 between 9:00 am - 12:00 pm.  

Bring the following:

  • Proof of income (last three months paystubs)
  • Proof of pregnancy (if relevant)
  • Proof of other insurance (if any)
  • Outstanding medical bills
  • Proof of Citizenship
    • Passport, or
    • Proof of birth (birth certificate) and
    • Proof of identity (driver's license)

Applicants will receive a Kroger gift card (while supplies last)
